This type of Form is intended for Beneficiaries - natural persons with permanent residence outside the Czech Republic and the Slovak Republic.

In respect to the form of distributions, i.e., the bank transfer, please note that this is an international payment transfer, and therefore it is required to disclose to the Trustee all information about that bank account, i.e. the account number, bank code variable and specific symbol (as required by respective banking institution), SWIFT code, name and address of bank, account name (first name and surname of account holder) and address of the account holder.

We recommend stating the Bank connection in the form of the IBAN (International Bank Account Number).

Distribution is depending on the Applicant's option implemented either in Czech crowns or U.S. dollars.
